Tracking Down Viral Load Non-Suppression: Using the Facility Community Encounter (FACE) Tracker

Traditional methods of follow-up are prone to delays and inaccuracies, leading to gaps in treatment adherence and compromised patient outcomes. The FACE Tracker addresses these challenges by providing a seamless solution that ensures patients are consistently monitored throughout their treatment journey.

The FACE Tracker system uses Kobo software on mobile phones to track patient information. At the health facility, Linkage Facilitators input clinical data like viral load, medication, and psychosocial support. In the community, NS Monitors update household and case management details. All this data is combined into a FACE report, which provides a complete overview of the patient’s health, including treatment history, clinical interventions, and follow-up actions. The report integrates both facility and community insights for comprehensive patient care.
