At the health facility, Linkage Facilitators input clinical data like viral load, medication, and psychosocial support. In the community, NS Monitors update household and case management details. All this data is combined into a FACE report, which provides a complete overview of the patient’s health, including treatment history, clinical interventions, and follow-up actions. The report integrates both facility and community insights for comprehensive patient care.
